Your First Appointment
Thank you for choosing Lake Placid Sports Medicine. Please review the following information to help you prepare for your first visit.
Paperwork
In an effort to maximize the efficiency of our registration process, we ask all new patients, as well as established patients with new problems, to please view, print and complete the following forms prior to your arrival at Lake Placid Sports Medicine.
What to Bring
In addition, we ask that both new and returning patients bring the following to their appointments:
Any of the forms that you completed at home
Your insurance card
A photo ID
Insurance referral, if required
Your co-payment. We gladly accept personal checks, cash, and most major credit cards.
Any X-rays, medical records, lab results, or other pertinent medical history
A list of current medications
What to Wear
Please wear clothes that allow you to show the injury site easily. Bring or wear shorts if you have a knee, hip or foot problem and a tank top (for women) if you have a shoulder problem. If being seen for a foot or ankle problem, bring your shoe inserts/custom foot orthoses. Please also bring or wear any other items that will help the provider evaluate your orthopaedic situation.
Arrival
Please be advised that Lake Placid Sports Medicine is a busy practice and wait times, as well as visit times, will vary depending on the day, office schedule, and nature of your appointment. We appreciate your cooperation and understanding.
Insurance
Lake Placid Sports Medicine participates with many insurance companies, including the following:

MVP Health Plan

Capital District Physician Health Plan (CDPHP)
For all patients covered by managed care insurances, it is expected that you the patient will obtain all necessary written referrals and authorizations required by your insurance to be seen in a specialist office. We will make every effort to assist you with this process however your insurance does not permit our physicians to provide care to you without authorization in place prior to your appointment. Your appointment will be rescheduled to the next available appointment if authorization has not been obtained.

Pay Your Bill
Lake Placid Sports Medicine offers patients the opportunity to pay bills online through a secure bill pay portal. To complete this process, you will need your recent invoice with the patient name and contact information, the account number, and the amount due. If you do not have this information, please contact Lake Placid Sports Medicine for additional payment options.
